Step 1

Print the User Sign-up Form and have it signed by your surgeon. Be sure to include all requested information, including an e-mail address so that your password can be sent to you. Once completed, please fax the form to 901-399-1307.

Step 2

Once your information has been processed, you will receive an email with your username and password from info@SmithNephewIDcards.com.
