Step 1
Collect the name and policy information from the patient’s insurance card.
Step 2
Refer to the PDF of the directory to determine which Specialty Pharmacy to use with the patient's health plan.
Step 3
Select the Specialty Pharmacy listed below that is covered by your patient's health plan.
Step 4
Complete the appropriate Specialty Pharmacy form and fax to the pharmacy. To complete the form online, click on the appropriate website link below or use the PDF download if available.
