It is easy to use SUPARTZ Direct

Step 1

Physicians need to register with the SUPARTZ Direct web portal
Complete and fax to 877-982-1086.

Step 2

Enroll and track patients in the SUPARTZ Direct program
Complete and fax to 888-399-0226. Patients can be tracked at http://www.supartzdirect.com/

How does it work?
This service can be used for both Buy and Bill orders and Specialty Pharmacy orders. Here’s how:

Buy and Bill

Step 1 - Send the completed Enrollment Form to SUPARTZ Direct or call the SUPARTZ Direct Hotline
Step 2 – Summary of benefits letter is sent back to your office once complete

Specialty Pharmacy

Step 1 – Send the completed Enrollment Form to SUPARTZ Direct
Step 2 – SUPARTZ Joint Fluid Therapy is delivered from the Specialty Pharmacy – no need to collect co-pay or bill insurance, the Specialty Pharmacy does this for you.
