These are the only available cards at this time. As Smith & Nephew launches new joint replacement technologies, new technology-specific cards will be added to the service.
Go to the Sign Up Link on the left hand side of this page and download the User Sign Up Form and the Clinic Authorization Form. Within a few days after sending them in, you will receive your new password and can begin using the service.
Patient ID Cards will be run every ten to fourteen days and shipped to you via the United States Postal Service.
They are welcome to use the cards as well, but all cards do feature the Smith & Nephew logo and mention our proprietary joint replacement technology.
Yes. In addition to having each patient sign an authorization form for your records, we have taken federally mandated steps to ensure the privacy and safety of all data gathered during the creation of these cards.
Smith & Nephew provides a digital form you may wish to print and keep on file for each patient. You can download the Patient Authorization Form here.
Once logged in, you can change your password to something that you can easily remember. Simply choose “Your Profile” in the top menu bar. You will be taken to a page where you can update your personal information. Now choose “Change your password.” Here, you are prompted to enter your current password and make up a new one.

Go to the online tool, and click on “Support” from the top menu. Enter your first name, last name and your email address. Click “Next.” On this page, fill in the first box with “I forgot my password.” In the second box, request a new password. A new password will be sent to you via email shortly.
