Cementless Modular Revision System
Prof. Dr. med. K. Zweymüller
The cementless hip system SL-PLUS® is the most frequently implanted artificial joint in primary care in Europe. The MODULAR-PLUS® system has been designed to complement the equally successful SLR-PLUS® system for primary revisions, and is indicated specifically for use in more difficult revisions. Here, too, the aim is to restore functionally-optimal stem-cup geometry. The modularity of the various proximal and distal implant components of different size is ideally combined with the basic principle of a stabile, conical, rectangular anchorage.
The SL-PLUS® and SLR-PLUS® stems are anchored in the proximal half of the femur. The MODULAR-PLUS® stem, however, is suitable for diaphyseal anchorage in the central and distal halves of the femur, depending on the severity of the damage and the load-bearing capacity of these areas of bone due to previous surgery. It is important that the third quarter of the diaphysis still has at least 10 cm of load-bearing cortical substance.
The system comprises 2 symmetrical components, a distal anchorage module and a proximal revision module, connected via a multi-step conical taper and secured by a conical screw. The distal anchorage modules are available in 12 sizes of different lengths and are subdivided into 3 groups of coupling diameters A, B and C. Each group can be combined through 2 proximal modules (standard and long). X-ray templates and trial prostheses support the use of the system before and during operations.
The double-contoured edges of the rectangular stem profile are self-tapping, yielding a conical seating over a large area, which in turn reliably prevents subsidence and rotation. The proximal revision module allows stepless adjustment of the individual anatomical antetorsion.
The MODULAR-PLUS® revision stem is recommended for use together with the CCG® cerclage system.
